Our Client: A mission-driven Alaska-based nonprofit is seeking a compassionate and resourceful Care Navigator to help individuals and families access critical health and social services. This role is ideal for someone who thrives on building trust, solving problems, and improving lives through personalized support and advocacy.

 
Your day-to-day:
As a Care Navigator, you will participate in shared office coverage, greeting guests and answering phones. As you gradually build your caseload, you will complete client intakes —ideally within one business day—schedule and confirm follow-up or consultation appointments, and educate clients and their support networks about available services. You’ll provide a smooth handoff to the staff members delivering services and, when needed, refer clients to outside agencies, healthcare providers, or other resources.
 
Your role also includes creating and maintaining reminder systems and follow-up schedules to ensure client satisfaction, conducting assessments to identify individual needs, preferences, and goals, and developing personalized plans that may include medical treatment, housing assistance, or mental health support. You will track client progress, address barriers, and adjust plans as necessary, while advocating for equitable and adequate services. Additionally, you will develop and maintain a comprehensive resource library, identify service gaps, and recommend new programs or partnerships to strengthen community support.
 
You will collaborate with service providers, monitor outcomes, and assist in improving policies and procedures related to navigation and support services. Outreach and community engagement are key components of the role, as you will represent ARA at events, workshops, and meetings to raise awareness and advocate for client needs. Other job-related duties will be assigned as necessary to support the organizational mission, and ensure clients receive exceptional, personalized care.
 
Physical Requirements:
This role involves frequent computer use and regular interaction with clients and community partners. The work environment is typically office-based, with occasional travel to events or meetings. May require sitting or standing for extended periods and occasional lifting of up to 25 lbs.
